McIlroy started the final round four shots adrift of overnight leader Adrien Saddier, but three birdies across the first five holes saw him quickly close the gap. He made a long putt at the turn to go into a share of the lead.
Lagergren made a stunning eagle at the par-five 16th to take a one-shot lead, but McIlroy responded with his first eagle of the weekend on the last hole to force a play-off.
McIlroy's victory at The K Club is his first since completing the career Grand Slam at the Masters in April. He expressed his excitement about the upcoming Ryder Cup at Bethpage Black in New York, where he will be joining Team Europe.
Before the Ryder Cup, McIlroy plans to compete at next week's BMW PGA Championship at Wentworth. The world number five described his Irish Open victory as exceeding all of his expectations.
